The Israel-Iran conflict continued into a seventh day after Iran's supreme leader rejected U.S. calls for an unconditional surrender and warned any military involvement by the Americans would cause "irreparable damage to them."

Meanwhile, Bloomberg reported senior U.S. officials are preparing for the possibility of a strike on Iran in the coming days, citing people familiar with the matter.

This comes as U.S. President Donald Trump continues to mull his response to the war.

An Iranian missile hit the main hospital in southern Israel early Thursday, wounding people and causing "extensive damage" but no serious injuries.

Israeli media aired footage of blown-out windows and heavy black smoke.

Other missiles hit a high-rise apartment building in Tel Aviv and other sites in central Israel. At least 40 people were reportedly wounded.

Israel, meanwhile, carried out strikes on Iran's Arak heavy water reactor, its latest attack on Iran's sprawling nuclear program.

Iranian state television said there was "no radiation danger whatsoever" and that the facility had been evacuated before the attack.

Meanwhile, Bloomberg reported senior U.S. officials are preparing for the possibility of a strike on Iran in the coming days. The report noted the situation is still evolving and could change.

Some of the people, according to Bloomberg, pointed to potential plans for a weekend strike.

Speaking to reporters, Trump declined to say if he had made any decision on whether to join Israel's campaign. Trump also addressed the matter of Iran's nuclear programme.

Trump said: "I have ideas as to what to do, but I haven't made a final. I like to make the final decision one second before it's due, you know, because things change, I mean, especially with war, things change with war. It can go from one extreme to the other. Yeah, well, I don't want to get involved either, but I've been saying for 20 years, maybe longer, that Iran cannot have a nuclear weapon."

"I've been saying it for a long time, and I think they were a few weeks away from having one, and they had to sign a document. I think they wish they signed it now. It was a fair deal. And now it's a harder thing to sign."

Amid the U.S. and Israeli concerns about Iran's nuclear programme, the country's state media outlet later posted a video of what appeared to be a dummy of a nuclear bomb. It included the cryptic message in the form of the word "maybe."

Meanwhile, Iran's exiled Crown Prince Reza Pahlavi said he and his supporters have a plan for the country should regime change be ushered in.

In a video message posted on social media, Pahlavi said they "are prepared for the first hundred days after the fall, for the transitional period and for the establishment of a national and democratic government, by the Iranian people and for the Iranian people."
